If you ever watched Clone High (the show Bill Lawrence did before Scrubs) there was a janitor on that show as well who was voiced by Neil Flynn. In the commentary for one of the seasons of Scrubs, Bill Lawrence said that The Janitor's real name was the same as the janitor's on Clone High. So it's evident that he gave J.D. his real name and told the other guy a fake one.
