The flares don't follow missiles, they have a high heat signature and heat seeking (AA) missiles will fly at them instead of the aircraft.

It doesn't guarantee it, missiles still sometimes hit it, but the Flares are a lot hotter than the plane itself and AA missiles go towards the highest heat signature where they were shot towards so they tend to go to the flares.

But sometimes they hit the plane either because the it went past the flares and hit the plane or that it reached its target anyway. But it usually works.
