Most piercers do pierce from tha inside out.. i found it weird too. When i got mine pierced they did it from inside out.

It's really not that bad at all. If you just breathe out while it's being done, that takes most of the pain away. It doesn't sting like your ears, it feels more like pressure. It looks worse than it feels.
