the outline of this story isn't really set in stone, so I guess it's up to the viewer and how they interpret what's going on.

here's what I see: After siwon gets his heart surgery, he feels drawn to the flower girl's store and he feels heart pains. he later realizes that the heart he got from the transplant belonged to hankyung, and that heart carried emotions and memories of his gf, the flower girl..

so that's why siwon feels so much pain cuz hankyung's heart is yearning for her..

No, Siwon's just guilty because he likes her. (I think...) He's getting flashback's of what happened over and over again. That's why in the end he gave her the necklace back, admitting that he was the one who did it.

That sucks though. You kill someone and they end up saving your life through a heart transplant.. So sad... Siwon must feel GUILTY.
