My guess is that it is something of a butterfly effect. If you mess with the past or the future, it will end up being twice as bad, in this case he has a heart attack and dies. My guess is that the book he is holding represents his life, birth date and death date. He rips a page out of it, something that he wanted to forget about, however you can not do that. Life does not work that way. You either have everything or nothing.
