Those fullface mask are dangerous, if your air really runs out, it's very hard to take that kinda mask off. Even in Finland's cold waters we use those simple masks, though water rarely is colder than +4°c.
Air can be "poisoned" in many different ways, without it being intentional. This is the reason to always have your cylinders filled at a reputable dive shop, and ask to see their air quality certificate as mentioned by HeatedUp89. I used to work at a dive shop and filled countless amounts of cylinders. The compressor was serviced regularly, filters changed, valves checked, seals, oil, etc. And have your cylinder visually inspected at least once a year (we won't fill otherwise)
Ah yes I had my tank filled once from a 'reputable' dive shop in Devon,UK ,And when I got in the water I noticed the air had a horrible metallic tasteand gave me a headache...so I aborted the dive....returning to complain later on that day the shopowner tried the air and denied it was their fault...then I saw the reason..the idiots had been melting down lead in a crucible to make weights in moulds...behind the shop under the compresser inlet!

He starts fiddling with his gear trying to figure out what's wrong. His body starts to convulse from asphyxia. Finally, he goes limp, sinking to the bottom, with his arms outstretched and motionless.
If the air was in fact poisoned, this is exactly why a CERTIFIED diver would have purged the tank to both SMELL and TASTE the air before entering the water...a simple case of user error...
Um, actually a certified diver probably wouldnt have smelt or tasted the carbon monoxide since it is an odorless gas. Instead before getting the air fill, the diver would have to ask at the shop/fill station for a air quality certificate.
