U don't get an achievement, but it should unlock the skull in the skulls menu... Make sure you are on normal or harder, cos it don't work on beginner.

Do you have 2 carry the skull 2 the end of the level because it wont give me the achievment when i get it ?

No you should get it as soon as you pick up the skull...

this is to ac3dan: some of the skulls dont give you achievments. the blind skull is one of them
