i can't help but think that the line, ""That that starts sweet ends bitter, and that which starts bitter ends sweet" was inspired by his ex-wife Madonna's song in Get Together, in which she sings, "if it's bitter at the start, then it's sweeter in the end".
It seems clear that the monologue is, literally, referring to the way that smoking is, in a way, a wolf in sheep's clothing, that draws us in, and kills us. We know it's dangerous, yet still we smoke for the luxury. The same for drugs. The same for power...and the same for that painting. The scene before this stated that "there's bad people on the streets after the painting" - yet Johnny still has it...he loves the painting's luxury, despite its potentially dangerous consequences. I think. :)
Capatalism is based on lies like that packet of cigs but yet we accept becuase it is there and we enjoy what comes from it ie.. material goods and wealth
I'm wondering how the two scenes connect, obviously there is supposed to be a connection, but in one scene there's dishonesty and punishment, and in Johnny's scene it's just an explanation of why life sucks.

Most people smoke because of the feeling and the reduction of stress, those might(maybe) create an addiction to it later in life, but some smoke because it makes them think about life in a different way, it makes them more creative, it makes them modest, it makes me honest, it makes me normal. Hes saying that not everything appears as what it is, something you might not consider special can turn out to make you really happy and that not everything regal or you suppose would make you happy really does in the end. This is why hes addicted to drugs cause it starts sweet but in the end is turning him into a drugee and also why he chooses to keep the painting cause its just a simple lucky painting but he belives it will bring him what he wants, and in the end it does.
