the ether is used because built engines have a much lower compression ratio. diesels are compression ignition so ether lights alot faster then diesel fuel alone allowing start up.
It's all about the injectors. I spent $1600 on my injectors. They don't make very much smoke. I'm making about 600hp with my Cummins. I started with a set of marine injectors that I bought for $300. Made similar power but killed my economy. They were fun in heavy traffic though. I was a real dick with those things in. I could black out a 1/2 mile stretch of 4 lane highway if there was no breeze.
No, they're not quite that simple. You've got the right idea but there is much more to it than that. I think what you're getting at is that you can push as much boost as possible and tuning is not a worry since a diesel cannot lean out the way a gas/spark engine will. Eliminating smoke on a mechanical diesel is all about injectors, fuel plate profile, AFC adjustment, having high enough lift pump flow to support the injection pump and so on...
Stepping up the turbo would actually make more smoke since it would take longer to spool. It would clean up when you built enough boost but that would take a while. Compound turbo's are a good solutions, But setting up the, fuel system is still a nightmare. Not to mention the problems with cold start that you get when you hotrod your diesel.
you obviously have no idea what you're talking about with cold start issues... the only time modded diesels have cold start issues is when you start lowering the compression so that you can add more boost without having super high cylinder pressures, my truck makes 425hp, i know multiple people with well over 1000hp cummins and duramax trucks... if i were you i'd check your intake air heater and see if you have any issues with that... a 600hp cummins is BARELY modified

I live in Phoenix. We are close to sea level, and have extremely hot tempuratures and I still have to use ether in the morning.

Do any of your trucks make 600hp? Keep in mind I am making that power with a 5.9 liter. My truck does low 12 second ET's.

Something is wrong then with your injectors or pump. There is no reason for even a 600 horse 5.9 to need ether.

Isuzu didnt move to Ohio, Duramax is a joint venture between Isuzu and GM. They are made by DMAX in Moraine, Ohio, look it up.
