Back in the beginning when Titans were first placed inside silos they were not very reliable. On average if I recall correctly one out of every four missiles test launched would fail either during launch or while in orbit. The damage cause was from silo pressure during launch.
This powerful, heavy ICBM shouldn't have been replaced with the light Minuteman ICBM. It should have been replaced with a heavy ICBM capable of carrying high yield warheads, not with the light Minuteman. The Minuteman should have been placed on mobile platforms like SAC and Curtis LeMay recommended. By making the light Minuteman the backbone of the US ICBMs fleet, the US allowed the Russians to take the lead in ICBMs with their SS-18 and SS-19 missiles.
The Titan II development began in the late 50s as an ICBM to upgrade capability from Atlas/Titan I ICBMs, and was placed into service beginning in 1962. Around that time, NASA order a number of units for the Gemini program and adapted them for manned spaceflight, with the first such launch in 1964 (or 65?). Apollo was the first US space program to use rockets developed from the ground up for manned flight.
@BigKwell actually the primary use was to deliver 2 men into space at a time as part of the gemini space program.. the use of the titan ii as a nuclear warhead delivery system was only its *intended, but never utilized* military purpose.

@DrVegasIII I use to be a moon landing hoax believer, but I've since changed my mind and think the moon landing did happen. My main argument was the flag flapping in the wind but that was explained away by science as were all my other arguments.

The first missiles (Atlas, Titan, etc) were ICBM missiles adapted to take persons into
space. The resoursefullness of NASA to convert and use these missiles for space
travel is a tribute to the engineering efforts of all the people involved.
