Well...I guess it only makes sense that the person who invites the other person out should be the one to pay. It would be a bit weird if the guy asks you out and is broke. That just means that they're sponging off of you. But then you can't let them be the only one doing the asking, coz then you're either too lazy to organise an outing (like me ^_^) or you're the one doing the sponging. The mutual asking works better if you're already in a relationship though.

Actually going out to get something to eat isnt that bad if you do it a couple of times instead of all the time alone. If you go by yourself it makes it easy for a person who is interested in you to come up to you, vs if you were with a group of friends because they would nervous and wont approach you.

It should always be the person who asks the other one on the date.
Having good manners is all about what you do yourself, so this is what I like to do:
--If I ask someone, I will pay (and tell them in advance to avoid any awkward moments)
--If someone asks me, I make sure to plan on paying (just in case) but also make sure they have an opportunity to pay (since they asked).
-----If the person asking me never pays for dates(and there is no good reason) I will talk to them (at a later time).
