You are exactly right, u can tell by the way he looks at the audience when he does it. I have heard him mention in several interviews that he scatts (spelling?) or mumbles in tune when he forgets the lyrics, and he admits that he forgets lyrics all the time, he also admits to having teleprompters sometimes to remind him, I love him in everything he does, even when he forgets the words he makes it his own and is completely unique! He is incredible to say the least.

Actually, they are women in the studio version of the song. They toured with the band for a couple years after Before These Crowded Streets ending in 2001. The Lovely Ladies were their stage name.
