To me the most difficult was Medal Of Honor - European Assault!

The "HERO" difficulty has no health packs,the enemys dropped ammunition disappears in seconds (so you have limited ammo in some levels),the enemy can take you down with one bullet,not to mention no checkpoints,no save points (so you have to finish the whole level),they give you extra life's,but if you die,you come back with half of a health bar full.
