Siam has an...elaborate design. Like a grown man but since he's strapped to that baby carrier thing it may mean that men are big babies inside and it's the wife who's have to take care of the big baby, but men hold women's fate on their shoulders that's why women can't stand on their two feet because of dependancy. Then men are such big bumbleheads sometimes that it takes women's effort to realize their ambitions, thus the hands as head. Oh, and remember, women: That means your hands are tied ;)

I guess the creators meant it to scare the protagonist more and bring more enlightenment to the players about what bothers those characters. It can be a little complicated, but looking at some of the monsters' appearances, it's not too hard to interpret what those monsters mean to the current protagonist. Also why the monsters vary from installment to installment :D
