It sounds like 'Georgia' to me. Not the original Hoagy Carmichael voicings of course, but that's what makes it so good. Just Google "Georgia" and ''chords" and you'll get the basic structure, the rest of it you'll have to figure out yourself. That's what makes good players. Maybe you could do your own rendition... something original and good in its own way.
