I fixed my exact same issue I fixed it by changing my energy saver from better battery life to higher performance! go figure - I researched this issue for hours days months and finally found a forum that had this solution and it has worked for two days now probably not ideal but hey has not happened since - touch wood good luck :)

MBP has 2 video cards. Switch to the more powerful one and the problem should go away.

I've found a partil FIX. Install gfxCardStatus and have your card switch modes while in use. The issue will go down to manageable levels.
