Actually, it is a picture of Kim Novak as she appeared in Alfred Hitchcock's film 'Vertigo.' The artist (Alexander Manasse) who was commissioned to paint the picture could not remember what she looked like in the film, so he painted the back of her head.

The film tries to be trendy by appealing to shock value - like her red hair and her screaming.
But one of the more subtle things is that she isn't sure he truly loves her, and yet she's trying to save him.
