Take more time and be more detailed in the patients History than in the byzantology of Physical examination.Remember about 70-80% of the informations you need lies in what the patient say!Furthermore a 10 min Echocardiographic evaluation will give u more data on his heart structure/valve functions than an hour of auscultation...

I was told similar, b/w 4 & 5th rib; but not to use the nipple as a landmark...which you already probably figured out since you viewed this.

the circular breast examination is most preferred because the "sweeping motion" or vertical motion is somewhat painful and awkward to some... just ask my return demo partner :P
