I don't think it has anything to do with Alheizmer's disease. I think its all make up. They did a great job making him look withered and aged. Pacino has looked much better in the films he has made since.

In my opinion this was the last great movie Pacino has done.

Not sure if this was intentional or not, but Pacino looks exhausted from the second he is on camera.
