The thing with Alzheimer's and her being the older lady to whom he tells the story is copied from the movie "The Notebook". It's not the first time Bollywood rips off american movies or copies certain things. They copied the movie French Kiss as well with their Pyar To Hona Hi Tha.

this movie was a mou tor jawab to kabhi alvida na kehna.. even though kajol ws ill ajay never left her alone..srk married to preity coz he loved her even after his accident she run the house and but he never thanked her more over he fell in love with rani.. ahhh shame full..and look at rani, so loving husband never asked her to do any thing not even a child.. but she fell in love with srk.. that movie ws just abt unfaithfulness

well it has a similar idea but its still very differnt from notebook. I mean him being a doctor and the time they spend together before marriage. Also they don't die at the end, or the scenes that show her behavior during the time she has the disease are differnt. I found the movie amazing, well directed- some of the special effects were cool.. and very original!
