In a nutshell, on red blood cells (the cells you receive in blood transfusion) there are antigens - substances which attack anything which does not pertain to your body. Like all parts of the body, those antigens are a product of your genetic material, meaning that the best matching blood would be your own, followed by that of a family member. On the same rationale, the further you deviate from your own gene pool, the bigger the chance of transfusion incompatibility.
