Really? Well, this is how I understood it... : Everything seems to be routine for J.D. now and as he listens to the song he realizes the whole hospital is in a cadence.

Then he starts daydreaming the patients singing and Kelso comes to ask him "Are you having a good time?" In his mad voice because the hospital can't have everyone dozing off.

I hope I cleared that up!
