Actually Mick Foley was never supposed to leave the stretcher, The chokeslam was improvised and he was never meant to be put through the cage, He disconnected both of his shoulders doing it.

the chokeslam undertaker did to mankind in the hell in a cell match was staged. You can see strings on mick foley while he is falling. The strings were attatched to the ceiling to his shoulders to prevent a hard landing on the mat. Also there was a "weak spot" in the cage where it breaks easily. Where Mankind landed had extra padding underneath the mat.

The people from top of the arena took the ropes(actually they were wires you cant see with your eye on tv) off after he was on a stretecher.
