Ono can say what he pleases, he didn't design her. Okamoto (director of Final Fight) said that she is a woman and was designed like one. When Capcom America complained about the violence against girls, Capcom Japan tried to dodge the issue by saying that she was a man.

In the latest invterview, held shortly after Poison's conformation, Ono stated that she's pretty much what you personally think of her most as. I personally think she's female, because a transsexual would have a bulge in shorts like those. Your turn. -_-

And technically, he/she wasn't 'born that way', they made themselves that way (fictional character or not). If so-and-so was born a guy, but wanted to be a girl instead, they'd have to make themselves that way. I don't think psycology has officially declared that state of mind as something you are 'born with' either. Please, I'm not trying to troll, I'm just saying what I think. If you don't agree, then that's fine.

But in Japan, she simply tucks her business away in order to look like a girl." Seemingly, Capcom thought a pre-op transsexual would be too taboo for conservative Western audiences.
