We can abstractly suggest that a rat is an embodiment of "bad"... and it leaves body after he took a pill. Then we can finalize "IT WORKS ! The rat is dead and is out of the body" =))))
Does this advert suggest that instead of putting some 1p a kilo generic powder, the fake pill maker goes out of his way and pocket to kill his customer..?
I think it is more trying to say that you dont know what is in fake pills. And that tests have found things like rat poison.... But there could be just placebo pills, or pills with far worse chemicals in!

The dramatization is of a danger that is very unlikely, it's like warning people off of cigarettes with a dramatization of setting yourself on fire.
And I'm sure if it was graphic people would react. But that wouldn't be effective, just as this, when people know the risk is a lie.
