I see what you mean about #5. I got that one on my first playthrough, I listened to it and it showed as unlocked. But now I'm replaying the level it says it's still locked. So I've picked it up again, it unlocked and I've definitely listened to it before... Unfortunately I didn't make note of the total skulls collected beforehand to see if it was already included.
