I think it's still easier to make real ones than go through the trouble of faking with actors with unheard voices. Richard and Sal prepare a lot for these calls, and probably know in advance they are calling a lady who is coughing, so they get some clips from the show and queue them up to play on demand. I wouldn't say they are fake just because they work out so well. See, they also make a lot you of calls you don't hear, that aren't worth playing or couldn't get permissions. Just what I think..
