it's Sugizo... He probably had one made, I doubt you can just buy three-necked guitars :) Btw, the middle neck is without frets. I'm not sure whether you can see it (the video is quite old, so quality is not perfect), but one can definitely hear it. Well, Sugizo is a violinist too, so it makes perfect sense.
