Just noticed that in the dialogue starting at around 4:12 , the subtitles are mixed up; when Thane talks, it still says Shepard.

@TheEmperor01 The problem is that anything mission-related like this in this game is event (condition) and trigger based. Although dialogue and subtitles are intended to be locked they also have a subtiming determining their display time. You could trigger one dialogue (voice) and another subtitle because of some basic overlap. It happens pretty darn rarely though. ;-)
