I think I do, its just that in the comment I responded to you completely missed the point, the song (yes I dont know for sure, but I you'll probably agree) is by no means an anti-smokers statement. That's not the writers point. I think it has to do with the fact that in a hospital is for curing people, and it is kind of sad ( ironic). People put something that's bad for them outside a buillding meant for healig people.

for all I know it could have been heroin addicts outside the hospital doors.
