I think he's just proving to us viewers that fermentation is taking place - that the CO2 produced has expelled the air above the fermenting wort, so the candle is snuffed out.
After some research, it appears that the kruesen head from the fermentation provides some protection as well. They still brew Kölsch beer with that type of open fermentation as evidenced in the Goffel Kölsch tour video.
Lots of large & small ale breweries in UK (& elsewhere) plus some wheat & lager breweries in Germany (Schneider Weisse & others plus some Alt & Kolsch brewers I think) still use open fermentation vats.
Breweries are pretty clean places, plus alcohol & CO2 kill some bacteria & hops themselves have a natural bactericide quality too.
That said, yes, most breweries have now moved away from using a cool-ship or the drip-down type cooler shown (I forget the term).

Good question. I think they chilled down the whole building to 12 degrees celcius to prevent growing mould and fungus.

The fermentation rooms were kept santized. Until recent decades, open fermenters were the norm for both ales and lagers.
