The medal pieces are held in place with a bar at the top of the kalimba, the varying lengths of the pieces vibrate at different frequencies, which produce a different pitch (amplified by the empty wooden box). The hole in the middle is like the whole of a guitar, but placing your finger in front of the whole will produce a sort of "wha" effect. Hopefully that clears it up for you.

The key can be changed manually by moving the keys (the long pieces) back and forth under the metal bar holding them all in place. They also range in the number of keys they have, some have 8, some have 15, and some have 17. Did that cover it?
