38-40 he's holding it with the tips of his fingers, pointing straight backwards away from the camera.. then uses his middle finger to flick it outwards quickly. a clever trick, but takes ALOT of practice to not flick it too hard, but at the same time, very fast.

It's real. The cigarette is straight to you view direction but he use his fingers to hide the cigarette then he use the middle finger to rotate the cigarette.

the last part is a thumb tip you can see him reaching in to his pocket just before he does it then he puts the tip in his hand then points his thumb at the camera and keeps it out of sight
