Your right foot is hitting the groung to late causing your pitching arm to not reach full velocity because your hips arent getting enough rotation with both feet planted which carries up through your shoulders.
Wow I just came off the same tear, but in my right shoulder. It kind of looks like your forcing it and dropping your shoulder. Understandable though, considering the compensation from lack of strength/confidence from time off.
hey killer... I understand that you just got off an injury, but when you get on the mound don't just throw. Take your time and concentrate. Relax your throwing arm and make it like a slingshot. This will reduce the tension that builds up on your shoulder and it will increase velocity. Also to increase velocity, Do not raise your knee and drop it and then go to the plate. Raise your knee and go from point a to point b. Knee comes up and goes directly to the plate. keep working hard.

Keep your glove up in front of you...the chest comes to the glove...that's why your head is going to the side. You have no "drag" line...visit the national Pitching Association's website for some good information. Without a frame-by-frame video it's difficult to tell what else may need looking at. Good luck!
i think that if your glove position is alittle lower in your balance position it would be easier for arm to get were it needs to be. it seems like your arm has to rush to the top of the release. i dont know your profile but that could be cause for injury. just maybe try it once and see how it goes

Right know I throw off a lower mound and Im probably younger and right know i might be able to hit 80, not to have a high ego or nething, I just turned 15 but I know ur injuried and thank god, knock on wood, this doesnt happen to me. NO u dont. You throw mid, maybe high 70's. Im in the WCAL, the hardest baseball league west of the mississippi, and im batting over 430. I also throw low to mid 80's (82-84), I dont really pitch though. I used to throw harder but I hurt my arm.
I'm have yet to be born and I can throw a medicine ball 41,000 mph.
