For this performance, I used a Boss loop station pedal. I am also using the PA systems built in delay and chorus through my guitar channel also.

Well that's up to you. There are a lot of people who cover song on youtube and never mention who the artist was. They just name the song. It's up to you how you want to handle it.

I don't think it matters how you credit it. Just be sure to mention that the song was originally written by Bob Dylan.

At the end of the day the song was written by Bob Dylan. So you are always doing a Bob Dylan cover. That is what Hendrix was doing when he recorded his version. He was covering Bob Dylan.
