this is definitely a uncommon scenario, and i bet the doctor is probably untrained in bug removal. the solution is obvious to someone watching this video, who is probably analyzing this in a stress-free situation, but when you're put in the hot seat doing something you've never done you may not be so quick to think on your feet. you'd probably approach it the way that is most familiar to you. for this doctor, that would be with surgical tools. give him a break, he's only human.
