Folks, many of you are too young to know about the Tenerife disaster, the worst commercial air disaster in history, in about 1977. Two fully-loaded 747's crash into each other on the runway, one of them at V1. Result: About 450 people burned to death. Cause? 100% pilot error in the KLM plane. Both planes in perfect condition--before they were torn apart & burned. The KLM pilot was a disgrace to the profession. Eternal shame on him!
Verdict: Flight engineer & pilot error in response to a minor problem on engine #4. I would cancel a reservation if I knew this captain was flying. This type of pilot means well & tries to do right, but can get passengers killed. Thanks to Boeing for building an airplane that will often forgive a confused or disoriented pilot. At the minimum, he needs complete re-training & physical/mental examination. Pilot fatigue is no excuse; his training should overcome this. Good job landing, though.

A lot of these problems seem to arise from the crew's understanding of the auto-functions of the plane. Might be a good thing since they would never have had the need before to use them, but considerably problematic for similar situations.
Rule # 1 - Someone must always fly the plane during ANY in-flight control problem. Not the auto-pilot. Left rudder to compensate the right roll tendency with # 4 engine loss, and trust your instruments(i.e. ADI).

Poor airmanship, lack of instrument interpretation, lack of systems knowledge, poor cockpit resource management, no use of emergency checklists, etc, etc. In summary a poorly trained crew almost caused a major catastrophe. We are trained to be in these situations i know the Flight simulaters are much much better now. i fly with American Airlines B-767-300ER and Delta A330-300 and i have seen how complicated these systems are so if there is one little mistakes it will be fatal !!

Seems pretty simple to me.. When you learn to fly IFR (no vis), you learn to trust your instruments, because your body gives you conflicting feedback. They had a minor engine problem, disengaged the auto pilot and lost control without the reference of the horizon. Those guys are totally incompetent to fly IFR. Scanning the instruments including the attitude indicator is a very basic IFR skill. I went through the training, it was always emphasized. MORONS!
You can never believe the NTSB or FAA. They are corrupt corporatist/government agencies. They cover up for falsified mechanic records and incompleted flight trainings. Almost always pilot error is given as the reason for a crash or near-crash!

he messed up childishly, but heroicly fixed it! Getting it out of such a dive is almost impossible, look at other Air crash investigations. Almost all end up fatal. Now with a broken rear??? Great job pilots.
Many airlines tried the manuever and attempt to land in their simulators with their best pilots after this incident. None of them can land the plane safely.

I knew it was pilot error as soon as they started wasting time trying to restart #4 instead of maintaining control of the aircraft. They induced a secondary stall by trying to go level while all three remaining engines were obviously in a compressor stall condition from the primary stall. The 747 has three redundant ADIs, and it was crazy to assume they were all inop just because they didn't like the data they were getting from it.

he messed up childishly, but heroicly fixed it! Getting it out of such a dive is almost impossible, look at other Air crash investigations. Almost all end up fatal. Now with a broken rear??? Great job pilots.

Many airlines tried the manuever and attempt to land in their simulators with their best pilots after this incident. None of them can land the plane safely.

@vincent3006 true but what we need to remember in part 1 the pilot didnt get any sleep, so his body was not fully alert and wasnt thinking properly

I knew it was pilot error as soon as they started wasting time trying to restart #4 instead of maintaining control of the aircraft. They induced a secondary stall by trying to go level while all three remaining engines were obviously in a compressor stall condition from the primary stall. The 747 has three redundant ADIs, and it was crazy to assume they were all inop just because they didn't like the data they were getting from it.
