Well, as with all songs, I think you predict it as you wish. If it means something different to someone else, that's ok. But the lyrics give it away. He's personifying HIV/ AIDS.

Have a look and try seeing from that perspective, eg 'No boat nor bridge, Nor crucifix can hold it back' It's a disease, can't escape or cure it. 'You saddened my freinds and claimed all my lovers' His freinds are upset, he may die, and his lovers all have it or have died from it. Etc :) have a look, it fits.
