I agree, it looks like it is a tough time pulling the retraction cords out. They are not intact cause the tip of the diamond burr has made a small cut in one of them. It is easy to pull them out though. No extra force is needed.

Regarding the removal of the excess at the cervical margin I make a complete stop with the dimaond burr a lot times to feel if there is any excess leftover at the cervical margin with the tip of the burr. Hope this was helpfull to you.
