Everything that you said doesn't work, works quite well for us up in Canada. We work we have care, we lose our jobs, we have care. We do not have to file papers with the gov't, we can use our health care anywhere in the country and choose any doc we wish. We can't however get things like implants done for free. That's a cosmetic thing :) But if you are sick, you are cared for. No Questions.
In Canada this is how it works. You have pain and go see your doctor. The results are you need your gallbladder removed. The doctor does NOT need approval from the insurance company. Your doctor calls the hospital to find out what times your surgery can be done.
(cont)The earliest is Next Wednesday 9 am which is in 7 days. Your doctor tells you a possible date for your surgery and you call your employee and ask if you can have the time off. Your employer says it is ok.
You contract your doctor and he calls the hospital to confirm. You show up at the hospital on the scheduled day and they perform your surgery. The doctor and the hospital sends the insurance company the bill.
The only papers we have to file are tax papers to get tax credits for drugs and med expenses. Which means money back in your wallet.

The insurance company can NOT deny any bill they receive unless it is fraud (over billing, service not performed, etc).
