You either end up with insurance companies that ignore the government and do their own thing, or you have the insurance companies join together and become a monopoly and a race to the bottom occurs. Only a true single payer system with an unregulated socialist method of distribution of health care for all can work. Pay doctors for doing their jobs, deduct from it for taking longer than average to heal, etc etc. No bureaucracy, just check for corruption.
