I don't know if anyone's already told you, but I'm gonna answer your question anyway.

It's called Full Moon Wo Sagashite.

If you haven't already watched it I recommend the japanese subbed. There's an English dub, but the main character's voice gets a little annoying (well to me it does).

The english title is usually just shortened to Full Moon. Watch it, it's REALLY good! :P

That wasn't Mitsuki lol. That was her mother, Hazuki. The guy who kissed her was Aoi (who is supposed to have blonde hair in the manga), Mitsuki's father. Takuto was younger back then because this was before Mitsuki was born. The guy Takuto spoke to was Keiichi Wakaoji, Mitsuki's doctor/producer in the manga (I don't know about the anime though because I didn't watch it and the manga is different... and better.).

Yeah, there's a bunch of differences in the anime about other things, i.e. how Mitsuki found out about Eichi, what Eichi's fate was, and the entire ending is very different. The anime's what introduced me to the series, but I far prefer the manga. ^^
