To start there was no Palastine or Palastinian entity before 1948, there was a British Mandatory province of Palastina, this name was borrowed from Roman history. There was a small Arab population, some nomadic Beduins, some Hashemite (Jordanian), some in small villages & towns Haifa Acre Jaffa Jerusalem.

The reason why the casualties of Palestinians have increased remains that Hamas, purposely has bases next to hospitals, schools, and mosques. It enjoys denouncing Israel when it accidentally kills the children that Hamas keeps close. Should Israel give Hamas leniency at the expense of Israel's citizens? Both sides have done terrible things to each other.
