@vlad007001 yeah the tree on his head has taken over his entire body thanks to the unpredictable nature of FEV mutagen. the tree grew rapidly spreading his organs throughout the ground via a massive root structure which is why he cant move and asks you to kill him. ghouls can live for hundreds of years but harold was one of the original who discovered the first hidden FEV vats with Richard Moreau who mutated into the Master the final boss of fallout 1

And if given the option to kill him, and he just reappears, the developers have to pull a mayor plot device as to WHY he is still alive.

But after all, the Courier probably won´t know him anyway.
