This isn`t the whole segment. In the actual episode after the elephant reacts like this they retest it with no mouse under the shell. The elephant walks by the shell and doesn`t even notice.

It appears it is myth that elephants are afraid a mouse will go up its trunk and suffocate it, evidently it will just go into a sneezing fit and blow the mouse out. Also I read a 20/20 article in which they went to an elephant trainer and asked what would happen if shown a white mouse and the trainer said not much but lets see. So he proceded to show the mouse to several elephants with little more than a bored reaction. So it apears its only a case of surprise and caution.

the real reason elephants are "afraid of a mouse is... well it has been proven that an elephant doesnt like to step on things.. like a line painted on the ground, the elephant will walk over it not on it. but since mice run around alot the elephant is afraid to step on it!
