Thats a great swing you have completed there - your backswing, tempo and followthrough are excellent. If you are having problems with distance, first and foremost check your grip. Your slice and hook problems may also be linked to an over active lower body on the downswing.
My slices are pretty much gone now. I've found that when I duck-hook the ball it's due to an arm swing where the arm out-races my lower body. If my hips turn properly ahead of my shoulders and arms, I might hit some block slices to the right, but they're usualy still playable.
I initially developed that to fight a big, weak and high slice to the right. Gradually the slice becomes a fairly straight ball flight, then a slight draw, and now it's turned into a really bad hook.
Very flat swing but when you are only 5'2" tall I guess that is acceptable.

you bring the club back real shallow, then you come over the top. p.s. that looked like a pop up; tee might be too high

Yeah you have to turn your shoulders or it will go wayyy left, i had the same problem...but now its fixed
