I believe there are two different Koakumas because Touhou fanon cannot decide if Koakuma looks 9 years old or 22 years old (or anything in between). Little kid Koakuma and adult Koakuma might as well be two separate entities.

Of course, adult Koakuma gets all the best fanart.

Actually, there sorta ARE two Koakuma - one in Stage 4 as midboss, and one in Extra stage as a dangerous enemy, and it has a darker and older-looking sprite.
