When Mario falls down a hole, the data indicating Mario's position doesn't go away, only his sprite disappears. Thus, the Mario data continues "falling" from the top of the screen in an endless loop. The way he "dies twice" involves the hammer collision data, which doesn't check to see if the sprite is actually there before making contact. This can be seen sometimes when trying this glitch, as you'll see the hammer that touched the Mario data "shift" in another direction.
I should also note that in order to see the hammer shift, you have to pause and unpause the game repeatedly after you get killed from enemy collision. The game is programmed to go to the lives display screen only after the death music has finished playing, but if you delay the death music long enough (about 5 seconds) everything frozen on the screen will start moving again.
I've recently found this glitch a few minutes ago while playing Super Mario Bros. on the 3DS.
