Sounds like the wheel or barrel or whatever the thing is that determines the order of the hammers is turned 1/4 out of sequence. The chime is starting with the 2nd half of the chime instead of the 1st half. Then, when you move the hand to the next chime-point, it's starting with what it should have finished with at the previous chime-point.
