knight rider was full of stuff like this!! The most common thing i can remember was that the bonnet (or hood...?!) not being closed properly at random times during a scene. A headlight not being fully up happened fairly often, and so did being able to see bumper over-riders of the camera car. I think. It's a long time ago now...

If i remember correctly, at the beginning of smokey and the bandit 1, the trans-am has a failed headlight bulb. The 70's and 80's were great :o)
